When a parent or grandparent lives in a skilled nursing facility, assisted living community, memory care, or convalescent home, getting to a notary is often not an option. Brent Hernandez comes to them, with a calm, patient appointment that moves at the resident's pace.

We work with families who need documents signed quickly, and with social workers, discharge planners, case managers, and administrators who need a notary they can call again and again. We cover care facilities across the Inland Empire, the Pomona Valley, and all of Southern California, including Pomona, Ontario, Upland, Montclair, Claremont, Rancho Cucamonga, Chino, and Fontana.

What We Notarize at Care Facilities

Powers of Attorney

Durable financial powers of attorney so a family member can pay bills, manage accounts, and handle property. See California power of attorney requirements.

Advance Health Care Directives

Health care agent designations and end-of-life wishes. Skilled nursing facilities have a special witness rule, explained below.

Trusts, Wills, and Deeds

Trust signing packages, trust transfer deeds, grant deeds, and transfer on death deeds. Estate planning signings are $250 flat.

Everyday Paperwork

Vehicle title transfers, bank and benefits forms, affidavits, and releases that need a notary stamp.

Important Rule

Skilled Nursing Facilities Need an Ombudsman Witness

This catches a lot of families by surprise.

What California Requires

Under California Probate Code 4675, an advance health care directive signed by a patient in a skilled nursing facility is not effective unless a patient advocate or long-term care ombudsman signs it as a witness. That is required even when the directive is notarized.

What to Do

Ask the facility's social worker to contact your county's Long-Term Care Ombudsman Program, and we will schedule the notary visit around the ombudsman. Read the full guide. Assisted living communities are not skilled nursing facilities, so this extra step normally does not apply there.

Before the Visit

How to Make the Signing Go Smoothly

ID

The resident needs a current government photo ID, or one issued within the last five years. No ID? Two credible witnesses who know the resident and have their own ID can often stand in. See what to do without an ID.

Awareness

The resident must understand what they are signing and want to sign it. If they seem confused or pressured that day, a notary has to stop, so pick a time of day when they are most alert.

Can't Sign Their Name?

A resident who cannot write a full signature can usually sign by mark, with witnesses. Contact us and we will plan for it.

Witnesses

Some documents need witnesses in addition to the notary. If you do not have any, we can bring witnesses for $50 per witness.

Fast Scheduling

Text or call (626) 248-0349 with the resident's name, room, and the documents. We confirm quickly, and same-day or evening visits are often available.

Respectful Visits

We check in at the front desk, follow your visitor policies, and keep the visit short and quiet. Client documents and journal entries stay private.

FAQ

Nursing Home and Assisted Living Notary Questions

Do you come to nursing homes and assisted living facilities?+
Yes. We come to skilled nursing facilities, assisted living communities, memory care, convalescent homes, and hospitals across Southern California, including evenings and weekends.
How much does a nursing home notary visit cost?+
Notary fees are $15 per signature, the California maximum, plus travel. Estate planning signings are $250 flat, and witnesses are $50 each if you need us to bring them. You get one all-in price before we come.
Does an advance health care directive signed in a nursing home need an ombudsman?+
Yes, if the resident is a patient in a skilled nursing facility. California Probate Code 4675 requires a patient advocate or ombudsman to sign as a witness, even if the directive is notarized. Assisted living is different and normally does not require this.
What if my parent has dementia?+
A notary can only proceed if the signer understands what they are signing and is signing willingly. Schedule the visit at the time of day they are most alert. If capacity is in question, talk to an attorney first.
What if the resident's ID is expired or lost?+
An ID issued within the last five years is acceptable. Without one, credible witnesses who know the resident and have their own ID can often be used instead.
